Turmeric and Your Medications: What to Check First

We sell a turmeric supplement. This article doesn't recommend one, ours included.
We're writing it because the search results for this question are bad in a specific, fixable way. Almost every page listing turmeric's drug interactions presents them as one flat list. A case report where a real patient's INR climbed to a dangerous level sits next to a result from a dish of breast cancer cells, in the same font, in the same tone of voice. A reader comes away either frightened of a spice or convinced the whole topic is overblown. Neither is right.
So here is the same information, sorted by what the evidence behind it actually is.
How to read an interaction list
Three tiers are worth keeping separate in your head.
Documented in a person. Someone took turmeric, something measurable happened, a clinician wrote it up. This is a small list and it deserves your attention.
Measured in a small group. A study gave curcumin to eight or twelve volunteers and measured a drug level going up. Real, but eight people is eight people, and a changed blood level is not the same event as a changed outcome.
Seen only in cells or animals. Curcumin does a great many interesting things to a cell line. Whether any of it happens in a human body at a dose a human would take is, in most cases, unknown.
This isn't our framing. The Welsh Medicines Advice Service, an NHS medicines information service, puts it directly at the top of its own turmeric page: the evidence is "mostly from in vitro (test tube) or animal studies so may not be clinically relevant in humans." Memorial Sloan Kettering's clinical monograph labels most rows in its own interaction table "unknown clinical significance."
One more distinction before the list. Turmeric in food and turmeric in a capsule are different exposures. Curry powder contains a few percent curcuminoids by weight; a supplement is a concentrated extract that can deliver fifty times as much in one dose. Everything below is about supplements.

The interactions that have happened to actual people
Blood thinners and antiplatelet drugs. This is the one to take seriously. Turmeric has antiplatelet activity of its own, so it is doing a mild version of what the medication is already doing. NHS Wales records a case report in which curcumin appeared to slow warfarin's clearance and a patient's INR rose "to a level associated with a serious risk of bleeding." A separate reported case describes 2.5 g of turmeric daily for five days measurably slowing clotting time in a 56-year-old woman on an oral anticoagulant. Warfarin has a narrow therapeutic window, which is exactly the kind of drug where a modest nudge matters.
Worth knowing: several things people take alongside turmeric push in the same direction. The NHS Wales list names angelica, clove, danshen, garlic, ginkgo, ginger, Panax ginseng and willow. If you're on an anticoagulant and taking three of those, turmeric isn't the interesting question — the stack is.
Tacrolimus and transplant medication. A documented case: a patient taking "15 or more spoonfuls" of turmeric daily for ten days developed acute kidney injury with a tacrolimus level of 29 ng/mL. MSKCC records a similar case of calcineurin inhibitor nephrotoxicity in a liver transplant patient. That's an enormous turmeric dose, but tacrolimus is a drug where levels are managed to a tight band, and curcumin inhibits the enzyme pathway that clears it.
Sulfasalazine. In eight healthy subjects, 2 g of curcumin raised plasma sulfasalazine 3.2-fold at therapeutic doses. The mechanism is inhibition of a transporter called BCRP. Eight people is a small study, but the effect size is not small.
Blood-sugar medication. Neerati and colleagues gave 475 mg of curcumin a day to eight people with type 2 diabetes on glyburide. Glyburide levels rose, post-meal glucose fell, and the effect was still measurable at 24 hours. Both halves of that finding belong in the same sentence: the interaction is real, and no patient in the study became hypoglycemic. Mention turmeric to whoever manages your diabetes. Don't expect a hypo.
Anti-cancer therapy. NHS Wales doesn't hedge this one, so neither will we: "Taking turmeric or curcumin while you are undergoing anti-cancer therapy is not recommended." Some of the underlying evidence is cell-culture work, but the drugs involved have narrow margins and serious consequences when their levels move, and the advice from a national medicines service is unambiguous.
The ones that look scarier in a list than they are on paper
Statins, hormone replacement therapy, loratadine, losartan, verapamil, midazolam, norfloxacin, doxorubicin, cyclophosphamide. All of these appear on turmeric interaction lists. Trace them back and most rest on animal or in vitro work.
The statin warning comes from animal studies showing increased rosuvastatin levels and a dose-dependent risk of muscle pain. NHS Wales still says "use statins cautiously with turmeric," which is fair. The HRT concern is explicitly theoretical: curcumin in large amounts "could interfere with HRT by competing for oestrogen receptors," based on in vitro research. The chemotherapy cell-line results are the weakest of all. Curcumin inhibited drug-induced cell death in cultured breast cancer cells, and MSKCC's own table marks each of those rows "unknown clinical significance."
"No human evidence yet" is not the same as "proven safe." It means nobody has looked properly. Mention what you're taking. Then get on with your day.
Two claims circulate widely and don't survive checking. The first is that turmeric reduces tamoxifen's effectiveness. MSKCC's monograph says close to the opposite, recording curcumin as protective against tamoxifen-induced liver disease. The second is that turmeric interferes with stomach acid medication. It appears on several high-ranking pages and on none of the institutional sources we opened for this article. We've left both out.
Before surgery, dental work, or anything with bleeding risk
Search this and you will get a number. Usually "stop two weeks before." Sometimes "three to five days." Every page states it with total confidence.
Here is where that number comes from. It traces to the discussion section of a 2023 Gastroenterology case report (Ghusn et al.) describing excessive bleeding during a colonoscopy in a patient taking a turmeric and fenugreek supplement. The authors suggested stopping at least three to five days, or up to two weeks, before a procedure with bleeding risk. That is a reasonable clinical opinion in a case write-up. It is not a guideline. No professional body, not the anesthesiologists and not the surgeons, publishes a turmeric-specific stop window.
We think you're better served knowing that than being handed a confident number with nothing behind it.
What to do instead is unglamorous and works:
- Write it on the pre-op form. Not "supplements." The actual name, and the actual dose in milligrams. Most people don't think of a capsule as something worth listing next to a prescription. It is.
- Say it out loud at the appointment too. Forms get skimmed.
- Let the surgical team set the date. They know the procedure's bleeding risk, what else you take, and how your clotting looks. That combination is not something a blog can price.
This applies to dental extractions and colonoscopies, not just operating rooms. The case that produced the whole two-week number was a colonoscopy.
Who shouldn't take a turmeric supplement at all
If you have gallstones or gallbladder disease. Curcumin makes the gallbladder contract, and not gently. In twelve healthy volunteers, doses of 20, 40 and 80 mg cut gallbladder volume by 34%, 51% and 72% within two hours. Note how small those doses are — far below a typical capsule. If there are stones present, that contraction is the problem. A perforation during stone passage has been documented in a man taking 1,200–2,000 mg a day.
The qualifier almost every page omits: turmeric does not cause gallstones or gallbladder disease. The risk applies to people who already have them.
If you form kidney stones. Turmeric contains oxalate. MSKCC's contraindication is direct: "Patients predisposed to kidney stone formation should consult their physicians before using turmeric supplements." A case of oxalate nephropathy in a 69-year-old man on chronic turmeric supplementation is in the same monograph; it stabilized after he stopped.
If you're pregnant or nursing. NCCIH states that turmeric supplements during pregnancy "may be unsafe," and that little is known about amounts above food levels while breastfeeding. This is an absence of study rather than a proven harm. It's still not a gap worth volunteering to fill.
If you've had liver injury from turmeric before. Documented cases of more severe recurrence on rechallenge exist. Don't restart.

The risk that isn't an interaction at all
Most interaction articles stop here. The largest documented risk with turmeric supplements isn't a drug interaction, and it doesn't appear on most of those lists at all.
According to LiverTox, the NIH's clinical liver-injury database, "turmeric appears to have become the most common cause of clinically apparent, herbal-related liver injury in the United States."
Both halves of that fact have to be stated together, so here's the other half: LiverTox estimates the incidence at roughly 1 in 10,000 to 1 in 100,000 people exposed, and describes it as "probably very rare." Turmeric sits at the top of that list partly because so many people take it. Rare and first are both true.
The detail is worth having. In ten cases from the Drug-Induced Liver Injury Network, patients were aged 35 to 71, 80% were women, 90% had hepatocellular injury, half developed jaundice, and median ALT was 1,140 U/L, roughly thirty times the top of a normal range. Most recovered within one to three months of stopping. One person died of multiorgan failure.
Three things from that series are genuinely useful:
- There is a genetic marker. Over 70% of the cases carried the HLA-B*35:01 allele, against 10–15% of controls. It's the same allele implicated in liver injury from green tea and Garcinia cambogia. You cannot know whether you carry it before something happens.
- The timing has a shape. Typical onset is one to four months after starting, with a range from a few weeks to eight months. The first few months are the window that matters, not year three.
- Feeling fine isn't evidence. In a separate series of eleven US cases, more than half were picked up on routine bloodwork with no symptoms at all.
NCCIH gives the practical version, and it's short. Stop and call your clinician if you notice fatigue, nausea, poor appetite, dark urine, or jaundice.
One more line from NCCIH deserves a place here, because the industry rarely repeats it: "Highly bioavailable formulations of curcumin, which enhance the body's ability to absorb the curcumin, may harm your liver." Enhanced absorption is a selling point on a great many labels. It is also, on the current evidence, where the reported liver cases cluster.
How much is too much

EFSA set an acceptable daily intake for curcumin of 3 mg per kilogram of body weight per day. For a 70 kg adult that's about 210 mg of curcumin a day, from every source combined: supplements, food coloring, the spice rack. Germany's Federal Institute for Risk Assessment built its 2021 opinion on that figure and titled it, bluntly, Acceptable daily intake may be exceeded. Its conclusion: intake "should not exceed 3 mg/kg bw and day in the long term," and prolonged exceedance "even slightly" warrants concern, "especially in sensitive individuals."
Now look at a shelf. Curcuminoid doses of 500, 1,000, even 1,500 mg per serving are ordinary. Against a 210 mg ceiling for a 70 kg adult, a good deal of the category is over it by several times, and marketed on being over it.
BfR goes one step further, and this part is about products like ours. Because piperine increases how much curcumin the body absorbs, BfR concluded that for enhanced-absorption products "it should be examined in each individual case whether the ADI derived can ensure a sufficient level of protection or whether the ADI should be adjusted downwards." In other words: adding black pepper may lower the real ceiling, not just raise the dose.
We'll put our own numbers against that, since it would be strange not to. The capsule we sell delivers about 142 mg of curcuminoids alongside 10 mg of piperine — roughly two-thirds of the 210 mg ceiling for a 70 kg adult before food and spice are counted, and appreciably less for someone smaller. We're inside it. We'd rather say that the ceiling is closer than this category likes to admit than let you find out from a German risk assessment.
What to actually do
Tell your pharmacist. It's free, takes five minutes, needs no appointment, and pharmacists are better on interactions than almost anyone else you'll speak to. Bring the bottle or a photo of the label.
Put it on every form. The FDA's own consumer guidance is that supplements belong in the conversation because "some supplements might interact with medicines or other supplements." The pre-op form is the one that matters most.
Know the four liver symptoms, and know the window. Fatigue, nausea, poor appetite, dark urine, jaundice. One to four months in is when to be paying attention.
Don't stack blindly. Turmeric plus fish oil plus ginkgo plus a daily aspirin is a different question from turmeric alone, and nobody has studied that combination.
Read the dose against the ceiling, not against the shelf. The biggest number on a label is a marketing decision, not a safety finding.
None of this is an argument against turmeric. Most people take it without incident. It's an argument for treating it like something that does something — which, given everything above, it plainly is.
FAQ
Is turmeric in food a problem?
For almost everyone, no. Culinary turmeric contains a small percentage of curcuminoids by weight, so a generous helping of curry delivers a fraction of what a capsule does. Every interaction and case report described above involves supplement-level doses. The one exception in the literature is the transplant patient who was eating "15 or more spoonfuls" of turmeric a day, which is a supplement dose arriving by spoon.
Can I take turmeric with ibuprofen or daily aspirin?
Ask your pharmacist first, particularly with daily aspirin. Aspirin at a cardiac dose is prescribed for its antiplatelet effect, and turmeric has mild antiplatelet activity of its own, so you're pushing in the same direction twice. MSKCC lists aspirin and ibuprofen in its turmeric interaction table. Occasional ibuprofen is a smaller question than daily aspirin, but it's still worth a five-minute conversation.
How long before surgery should I stop taking turmeric?
There is no official answer, and anyone giving you a confident number is quoting a case report. Suggested windows run from three to five days up to two weeks, but no professional body publishes a turmeric-specific guideline. Write the supplement and its dose on your pre-op form, mention it at the appointment, and let the surgical team set the date based on your procedure and everything else you take.
Does black pepper make turmeric riskier?
It changes the exposure. Piperine increases how much curcumin the body absorbs, which is why it's added. Germany's BfR concluded that for products with enhanced absorption, regulators should consider whether the daily ceiling needs to come down, and NCCIH notes that reported liver injuries cluster in high-bioavailability formulations. You don't need to avoid piperine products over this. You do need to read the curcuminoid number on the label instead of assuming more absorption is straightforwardly better.
What are the signs of turmeric-related liver trouble?
NCCIH lists fatigue, nausea, poor appetite, dark urine and jaundice. Stop taking the product and contact your clinician if any of those appear. Onset is typically one to four months after starting. In one case series more than half of the people affected had no symptoms and were found on routine bloodwork, so if you take turmeric daily and have blood drawn for other reasons, it's worth mentioning that you take it.
Do I need to stop turmeric before a dental extraction?
Tell your dentist you take it and let them decide. Extractions carry bleeding risk, and the case report that generated the widely quoted two-week figure involved a colonoscopy, not major surgery. The rule is the procedure's bleeding risk, not whether it happens in a hospital.
